THINGS TO BRING WITH YOU TO ALL VISITS:

 

  • Your current insurance card

  • Any forms you need us to complete

  • Your co-pay (unless you're having a Physical)

  • All medications you are taking, both prescription and non-prescription (over-the-counter, vitamins, supplements)

  • Any recent test results or medical records you feel would be helpful

  • Under 18 years old: complete immunization records

  • You may want to jot down key items of your medical history just to make sure you don't forget anything.

Please plan to arrive about fifteen minutes before your scheduled appointment time to take care of paperwork, though there's not much.
